Fraud and Abuse

You can play an
important role in helping Medicare identify
fraudulent providers.
Here are some ways you can help:

Report any suspected fraud to:
1-800-MEDICARE (1-800-633-4227) or
Report Fraud Online

Write down all medical appointments on a
calendar or in a notebook. For each date of
service, include:
- the name of the physician, nurse,
or therapist you saw
- where you received the service,
for example: in the doctors office, the
clinic, etc.
- the services you received
- if blood or other samples were
taken
- the medications you received and in
what form (shots, pills, inhaled)

Review your Medicare Summary Notice
(MSN) and compare it to the information in
your calendar or notebook.
